Lead Service Line and Fixture Mapping Project — Required by March 9, 2017

Ohio House Bill (HB) 512 requires community and nontransient noncommunity public water systems to identify and map areas of the distribution system that are known or likely to contain lead service lines. These systems are also required to identify and provide a description of the characteristics of buildings served by the system that may contain lead piping, solder, or fixtures. Single building community and nontransient noncommunity water systems are required to map areas of the system that have solder, fixtures, and pipe containing lead.
With the map submission, HB 512 requires the following are included: (1) a map with details as described for distribution systems or single buildings, (2) a list of the sampling locations (see Ohio Administrative Code Rule 3745-81-86 (A) for requirements; the information from the SMP ID project described in the paragraph immediately below can fulfill this requirement) and (3) a narrative including descriptions, record information, or other details not listed on the map. Note, if the water system doesn’t have any lead service lines, documentation must be provided to show this is the case. Completed maps, sample location lists and narrative must be submitted to your Ohio EPA District Office no later than March 9, 2017. Copies of the maps must be submitted to the Ohio Department of Health and the Ohio Department of Jobs and Family Services no later than March 9, 2017. Lead and Copper Sample Monitoring Point IDs
As part of the lead mapping project, PWSs were required to complete a template listing lead and copper sample sites. If your PWS has not submitted, please email the completed template to email@example.com as soon as possible. The SMP IDs will follow a standard format LC201, LC202 etc. and will be associated with specific consumer addresses within the PWS and specific contact information (phone numbers, email addresses) for the consumers at a sample site. To complete this work, Ohio EPA is asking PWS Owners and Operators to follow this link: http://epa.ohio.gov/ddagw/pws/leadandcopper.aspx and download the Excel file template (SMP ID Template). PWSs should complete a row for each lead and copper sample site within the distribution system and include all of the available information, but at minimum complete the required columns. Detailed instructions on completing the template are also provided on the webpage. After completing the SMP ID Template, please save a copy for your record (and future updating) and send by email to firstname.lastname@example.org with “Lead and Copper SMP IDs” in the subject line. In early 2017 Ohio EPA will notify PWSs and certified labs to begin using the new SMP IDs with lead and copper sample submission.
